PLEASE SUBMIT THE ONLINE REGISTRATION FORM BELOW.
 

Class placement is based on a first come first served basis.
To confirm your spot you must make full payment.

USE THE REGISTRATION FORM BELOW to hold your place, and then mail check or deliver cash or check to:

  • Downtown Dance
  • 223 NW 2nd Street 
  • Corvallis, OR 97330
  •  Mail slot to the left of our door 24 hours a day.  (2nd and Van Buren)
*Venmo payments are accepted with an additioanl 2% bank fee.   @DowntownDanceCorvallis

I understand that any physical activity brings a risk of injury and group classes bring a risk of exposure to Covid-19. With this awareness, I am participating or allowing my child to participate in this dance activity with Downtown Dance LLC.  My signature below states that I will take responsibility for the medical costs of the dance participant listed above should there be need for any.  Furthermore, by signing this form I hereby agree to indemnify and hold harmless Downtown Dance LLC, its property, and any instructors against any/all claims, damages, demands, liabilities and costs incurred including attorney’s fees as a result of any injury or illness caused by this activity.
